WHY SHOULD I JOIN?
• Smoking is the leading cause of preventable death in the USA, and places you at an increased risk for heart disease, stroke, fertility issues, cancer, and other health complications.
• Upon completion of the program, employees save $86/month by removing the tobacco surcharge on your medical contributions AND you will be refunded surcharge payments made since the beginning of the year. That’s $1040 in your pocket, not to mention the average $2000+ saved annually on cigarettes.

• Complete the program at your own pace, on your own time.
• Upon completion, Samuel will automatically remove the tobacco surcharge from your future contributions and refund surcharge payments previously made during the year.
Tobacco-User
An Employee who uses a Tobacco Product or Products four or more times per week within the past 6 months by legal users of tobacco products (generally those 18 years and older) – as defined by Centers for Medicaid and Medicare Services (CMS)
Tobacco Product
As defined by the US Food and Drug Administration, includes any product made or derived from tobacco intended for human consumption (except products that meet the definition of drugs), including, but not limited to, cigarettes, cigars (including cigarillos and little cigars), dissolvables, hookah tobacco, nicotine gels, pipe tobacco, roll-your-own tobacco, smokeless tobacco products (including dip, snuff, snus, and chewing tobacco), vapes, electronic cigarettes (e-cigarettes), hookah pens, and other electronic nicotine delivery systems.
